om scans van je longen automatisch te analyseren, om artsen te ondersteunen daarbij.

En was het vanaf het begin al dat doel?

And was that the goal from the beginning?

Nou, waar Tirona eigenlijk vandaan komt, is dat ik heb ooit AI gestudeerd.

Well, the origin of Tirona is that I once studied AI.

Daarna in de wetenschap ben ik eigenlijk hier ingerold.

After that, I actually rolled into science here.

Dus vrij lang geleden kwam op dat als je nou CT-scans maakt van je longen,

So quite a while ago it came to light that if you do CT scans of your lungs,

And I have done that for a long time as a scientist in the Netherlands and in America.

En daar was een soort frustratie van mij, dat als je dat in de wetenschap doet,

And there was a sort of frustration on my part that if you do that in science,

dan maak je allemaal dingen die eigenlijk heel erg kunnen helpen voor patiënten

then you create all kinds of things that can really help patients.

en voor artsen voor hun dagelijks werk, maar die komen daar niet terecht.

and for doctors in their daily work, but they do not end up there.

Omdat je het doet om aan te tonen dat het kan en dan ga je weer naar het volgende.

Because you do it to show that it can be done and then you move on to the next thing.

Dus ja, Tirona is inderdaad wel echt opgericht met alles wat daar heb geleerd.

Want wat was de frustratie?

Nou, de frustratie is vast voor mij persoonlijk dat als je het doet als wetenschapper,

Dus dat je bijvoorbeeld een arts efficiënt kunt laten zijn

As a scientist, your goal is to help advance the world by demonstrating where it can go.

En dat is heel erg leuk, maar als je in zo'n toegepast gebied zit,

And that is very fun, but if you are in such an applied field,

dan wil je uiteindelijk het liefst ook dat wat je gemaakt hebt gebruikt wordt.

Then you ultimately want what you've created to be used.

En wanneer wist je dat je die AI kon toepassen op deze scans?

Nee, eigenlijk tijdens mijn studie.

Dat is een promotieplek waar je dan AI ging toepassen op medische beelden van longen.

Van een hele abstracte studie naar echt iets toepassen.

So that's how I ended up there once...

En dat promotieonderzoek...

And that PhD research...

Dat promotieonderzoek was in Amerika?

Was that doctoral research in America?

Nee, dat was in Utrecht.

No, that was in Utrecht.

In Utrecht, oké.

In Utrecht, okay.

Ja, dus dat hebben we in Utrecht gedaan.

Yes, so we did that in Utrecht.

En tijdens de promotie was het gewoon heel leuk, maar ik heb dus heel veel geleerd.

And during the promotion, it was just very enjoyable, but I learned a lot.

En na mijn promotie wist ik niet zo heel goed wat ik wilde,

And after my promotion, I didn't really know what I wanted.

maar ben ik naar Amerika gegaan.

but I went to America.

Eigenlijk omdat daar een groep zat die het analyse van die CT-scans

Actually, because there was a group over there that was analyzing those CT scans.

ook in de wetenschap al direct toepaste bij onderzoek naar nieuwe medicijnen en behandelingen.

also applied directly in science in research into new medicines and treatments.

Dus naar die groep ben ik toen verhuisd, dus naar Los Angeles.

So I moved to that group, so to Los Angeles.

En dat was naast heel leuk, ook heel nuttig.

And that was not only very fun, but also very useful.

En daar eigenlijk heel snel, toen ik daar werkte, want dat was ook bij UCLA,

And actually very quickly, when I was working there, because that was also at UCLA,

dus ook bij de universiteit, ben ik eigenlijk na een paar maanden al daar

Want wat er vaak gebeurt is, je maakt een nieuwe behandeling voor iets in de longen.

Because what often happens is, you develop a new treatment for something in the lungs.

Dus dat ben ik als wetenschapper gaan doen.

So that's what I started doing as a scientist.

En toen kwam die frustratie weer terug.

And then that frustration came back.

En dan heb ik eigenlijk met het Radboudumc samen...

And then I actually have it together with Radboudumc...

en uiteindelijk wat ik deed, was dan overdag wetenschap,

and eventually what I did was science during the day,

s'avonds zelf met artsen en anderen die alles wat wij maakten wilden hebben,

in the evenings with doctors and others who wanted everything we made,

dat dan ook nog toepassen. Dus gewoon twee dingen naast elkaar.

then also apply that. So just two things next to each other.

En op een gegeven moment kwam er iemand naar me toe,

And at a certain point, someone came up to me,

bij een beurs, en die zei, hé, wij hebben hier 20.000 CT-scans

at a fair, and they said, hey, we have 20,000 CT scans here.

en die heeft een bedrijf geanalyseerd en dat is mislukt.

and that analyzed a company and it failed.

Ja, de spin-off was wel een idee dat er een soort van spontaan ontstond in mijn hoofd.

Yes, the spin-off was indeed an idea that kind of spontaneously came to my mind.

Het was niet dat de hele tijd mijn doel was.

It wasn't that my goal the whole time.

Eerst dacht ik, ik ga dan bij een bedrijf werken, van weet je, het impact maken.

At first, I thought I would work for a company, you know, making an impact.

Maar ik vond ook, alles wat we zelf deden en wat je in de wetenschap was zoveel verder

But I also found that everything we did ourselves and what you had in science was so much further along.

dan wat heel veel bedrijven deden, dat dat een beetje samen kwam tot, ja,

then what a lot of companies did, that it kind of came together to, yes,

ik wil, als ik de patiënt wil bereiken, dan ga ik dat via een spin-off doen.

I want to reach the patient, so I will do that through a spin-off.

En deed je dat toen helemaal in je eentje of had je al partners?

And did you do that all by yourself back then or did you already have partners?

Nee, ik heb het sowieso opgericht samen met een iemand anders van het UMC.

No, I set it up anyway together with someone else from the UMC.

Die is wel altijd bij het UMC blijven werken, maar dat hebben we wel samen gedaan.

She has always continued to work at the UMC, but we have done that together.

En we hebben eigenlijk...

And we actually have...

Eigenlijk omdat die organisatie met die 20.000 CT-scans is ook meteen een klant geworden.

Actually, because that organization with those 20,000 CT scans also immediately became a client.

Dus we hadden eigenlijk meteen ook wel geld om ook mensen aan te nemen.

So we actually had money right away to hire people as well.

Dus we zijn eigenlijk, we zijn niet de route gegaan van investeerders zoeken of iets dergelijks.

So we actually haven't gone the route of seeking investors or anything like that.

We hadden meteen, daarnaast hadden we toen uit mijn eigen netwerk meteen nog een klant.

We immediately had a client from my own network as well.

So actually, with customers, we immediately hired a small group of people and got started.

So we received that assignment for the 20,000 scans and all the follow-up.

Het is nog steeds een klant van ons, want die blijven maar scannen.

En we hadden het ooit getest op 100 scans van één studie met dezelfde.

And we had once tested it on 100 scans from one study with the same.

So they said there, okay, we are just going to scan a lot of people from all stages of CPD.

En over hele lange tijd, om uit te vinden, wat gebeurt er nou met die mensen?

And for a very long time, to find out, what is happening to those people?

En wat gebeurt er in hun longen?

And what happens in their lungs?

En kunnen we ook de genetica daarachter vinden, zodat we medicijnen kunnen gaan maken?

And can we also find the genetics behind it, so that we can start making medicines?

Dus daar zou je misschien een medicijn voor luchtwegen kunnen maken.

So maybe you could make a medication for the respiratory system there.

Kunnen we dan ook vinden wat daar genetisch, of daar een genetische component aan zit.
