Sie sind der Gesundheitsexperte beim Verbraucherzentrale Bundesverband.

You are the health expert at the Federal Association of Consumer Centres.

Sagen Sie doch mal, was machen Sie da? Mal so eine Vorstellung.

Just tell me, what are you doing there? A sort of presentation.

Wir machen Politik für Verbraucher in allen möglichen Bereichen.

We engage in politics for consumers in all possible areas.

Unter anderem auch im Gesundheitsbereich.

Among other things, also in the healthcare sector.

Aber wir beschäftigen uns auch mit Energie, mit Finanzdienstleistungen.

But we are also involved in energy, in financial services.

Mit Ernährung. Das fällt auch in meinen Bereich zum Beispiel.

Das ist unser Job. Wir vertreten die Interessen der Verbraucherzentralen,

Sie sagen, 170 macht die GKV,

Ich glaube, beim Staat, da müsste ich jetzt in die Zahlen reingucken,

sind es etwa ein Zehntel,

Also ein Bruchteil davon.

Das ist auch bei den Einnahmen der gesetzlichen Krankenversicherung bemerkbar.

Denn die Einnahmen basieren ja im Moment auf den Lohneinkommen im Wesentlichen.

Und wenn das weniger wird,

wenn überhaupt der Anteil der Erwerbseinkommen

an der gesamten Wirtschaftsentwicklung geringer wird, das ist so,

Gleichzeitig steigen die Ausgaben im selben Umfang,

Und das ist auch typisch für die Gesundheitsexperten.

And that is also typical for the health experts.

Die machen das auch gerne,

They also like to do that.

weil sie dann den Leuten, die eigentlich mal Überblick haben wollen,

because then they can provide an overview to the people who actually want to have one,

genau diese Erwartungen nicht erfüllen.

Insofern ist es richtig, dass sie einhaken.

In that respect, it is correct that they intervene.

Ich mache dann auch gleich mal das Fass ganz groß auf

Wenn man sich dort anschaut, was dort so abgeht,

dann stellt man halt fest, ein System,

then you just realize, a system,

was weitgehend von privater Wirtschaftsstruktur geprägt ist.

which is largely characterized by a private economic structure.

Im Detail kenne ich das jetzt genauso wenig wie,

In detail, I know just as little about it as,

wie ich behaupten könnte, ich hätte jetzt unser System verstanden.

as I could claim that I now understand our system.

Offensichtlich erkennen aber auch selbst die Amerikaner,

Apparently, even the Americans themselves recognize that,

dass sie gegenüber den anderen Systemen ja in irgendeiner Form im Rückstand sind.

that they are indeed lagging behind the other systems in some way.

Da gibt es ein sehr krasses Gefälle zwischen Kanada und den USA.

There is a very stark contrast between Canada and the USA.

Aber wenn man jetzt mal so weltweit blickt,

vielleicht sagen wir mal jetzt westliche Länder,

maybe we say now western countries,

westliche Staaten, Industrieländer,

Western states, industrial countries,

so das, was wir auch, sagen wir mal, kennen und kulturell auch für vergleichen,

so that which we also know, let’s say, and can compare culturally,

vielleicht sagen wir mal westliche Länder,

maybe let's say Western countries,

wie viele Systeme bilden sich da eigentlich so im Groben heraus?

How many systems are actually emerging roughly?

Also wie unterschiedlich sind eigentlich die Ansätze?

So how different are the approaches, actually?

Sind eigentlich alle irgendwie gleich?

Are all people somehow the same?

Sind die Unterschiede nur marginal?

Are the differences only marginal?

Oder lassen sich da einfach ganz grundsätzliche Unterschiede festmachen?

Or can some fundamental differences be identified there?

Und wo steht Deutschland?

And where does Germany stand?

Also es gibt drei wesentliche Typen.

So there are three essential types.

Der eine Typus, wenn ich jetzt nochmal die USA mit reinnehme,

The one type, if I include the USA again,

ist der, jeder sorgt,

is the one, everyone cares for,

der sorgt für sich, beziehungsweise der Arbeitgeber sorgt für eine Art privater Krankenversicherung.

He takes care of himself, or rather, the employer provides a kind of private health insurance.

Und wer das irgendwie nicht schafft, wer da nicht reinfällt und trotzdem krank wird,

And anyone who somehow cannot manage that, who doesn’t fall into it and still becomes ill,

für den sorgt der Staat.

The state takes care of that.

Das ist das System in den USA.

That is the system in the USA.

Also Medicaid und Medicare, das ist für die Älteren und für die,

So Medicaid and Medicare, that is for the elderly and for those,

die krank werden und keine Krankenversicherung haben,

those who become ill and have no health insurance,

die werden dann versorgt.

They will then be provided for.

Aber die werden immer erst versorgt, wenn es ganz schlimm wird.

But they are always taken care of only when it gets really bad.

Also wenn sie schon in der Emergency angelangt sind, also in der Notfallaufnahme.

So if they have already reached the emergency room, that is, the emergency admission.

Und deswegen ist das amerikanische System auch so teuer,

And that is why the American system is also so expensive,

weil die Versorgung zu spät einsetzt für gerade die Bevölkerungsgruppen,

because the supply starts too late for precisely the population groups,

die eigentlich medizinischer Versorgung bedürften, die Ärmeren.

those who actually need medical care, the poorer ones.

Wir wissen, das ist in fast allen Gesellschaften, auch in unserer so.

We know that this is the case in almost all societies, including ours.

Je ärmer man ist, desto höher ist das Risiko, krank zu werden und frühzeitig zu sterben.

The poorer one is, the higher the risk of becoming ill and dying prematurely.

Das heißt, da müsste eigentlich frühzeitig auch medizinische Versorgung,

This means that there should actually be early medical care as well.

auch Prävention, aber auch medizinische Versorgung einsetzen.

Also use prevention, but also medical care.

Und wenn das zu spät passiert, wird es richtig teuer.

And if that happens too late, it will get really expensive.

Deshalb ist dieses System in den USA das teuerste,

Therefore, this system is the most expensive in the USA,

und zugleich das ineffizienteste,

and at the same time the most inefficient,

weil gleichzeitig eben an die 50 Millionen gar nicht versichert sind.

because at the same time around 50 million are not insured at all.

Das ist auch der Grund, weshalb die Regierung jetzt da rangegangen ist,

That is also the reason why the government has now taken action.

weil es auch wirtschaftlich, volkswirtschaftlich ein Wahnsinn ist, was da passiert.

Weil am Ende zahlt der Staat ja auch.

Oder die Staaten sind es in dem Fall meistens.

Das ist der eine Typus.

Der ist aber sozusagen außer in den Entwicklungsländern und in den USA eher untypisch.

Dann gibt es auf der anderen Seite,

die Systeme, die ein staatliches Gesundheitswesen haben,

also Skandinavien, Großbritannien,

wo also das Gesundheitswesen aus Steuermitteln finanziert wird.

where, therefore, the healthcare system is financed by tax revenues.

Der Staat ist verantwortlich, der Staat kann das steuern.

The state is responsible, the state can control that.

Die Ärzte sind auch angestellt in der Regel.

The doctors are usually employed as well.

Beim Staat dann direkt oder wie?

With the state then directly, or how?

Beim Staat oder bei den Kommunen, je nachdem, wie das organisiert ist.

At the state level or with the municipalities, depending on how it is organized.

Und es gibt aber auch eine klare Zuweisung, meistens regionale Zuweisung.

And there is also a clear assignment, mostly a regional assignment.

Ich kann nicht einfach den Arzt frei wählen,

I cannot simply choose the doctor freely,

sondern ich muss dann dahin gehen, wo ich eben registriert bin, gemeldet bin.

but I have to go to where I am registered, where I am registered.

Es gibt in der Regel auch Zugangsschwellen zur ärztlichen Versorgung.

There are usually barriers to accessing medical care.

In Schweden ist das zum Beispiel so, da begegnen sie erstmal nicht dem Arzt,

In Sweden, for example, you do not first encounter the doctor.

sondern da werden sie zunächst mal von einer allerdings sehr gut ausgebildeten Pflegekraft befragt, versorgt.

but there they will first be questioned and cared for by a very well-trained nursing staff.

Das haben wir hier in Deutschland so nicht.

We don't have that here in Germany.

Die Ausbildung ist auch gar nicht entsprechend.

The training is not appropriate at all.

Aber da gibt es entsprechend auch viel weniger Arztbesuche.

But there are also correspondingly many fewer doctor visits.

Also deswegen muss man immer aufpassen, wenn so Arztbesuche international miteinander verglichen werden.

That's why one always has to be careful when comparing medical visits internationally.

Das ist etwas schwierig, weil wir hier in Deutschland ein Netz von Ärzten haben, zu dem sie immer gehen können.

This is somewhat difficult because we have a network of doctors here in Germany that they can always go to.

Das ist eben in anderen Ländern nicht so.

That's just not the case in other countries.

Die staatlichen Gesundheitssysteme sind so über den Daumen in der Regel weniger teuer, effizienter und die Leute sind mindestens genauso zufrieden.

The public healthcare systems are generally less expensive, more efficient, and people are at least as satisfied.

Also die haben unterm Strich keine so schlechte Bilanz.

So they don't have such a bad record after all.

Das Problem ist natürlich immer, dass der Staat von Jahr zu Jahr entscheidet, wie viel gebe ich aus.

The problem is, of course, that the state decides year by year how much to spend.

Also sie hatten jahrelang in Großbritannien eine Unterfinanzierung des Gesundheitssystems, des National Health Service.

So they had years of underfunding of the healthcare system, the National Health Service, in Great Britain.

Der war sehr beliebt in der Bevölkerung, hat aber sehr an Ansehen verloren, weil eben die Leistungen immer schlechter wurden.

He was very popular among the population but lost a lot of prestige because the performance kept getting worse.

Und das hat dazu geführt, dass viele Briten sich privat zusatzversichert haben.

And this has led to many Britons taking out private supplementary insurance.

Die, die sich das leisten konnten.

Those who could afford it.

Und da haben dann viele, auch übrigens deutsche Ärzte,

And then many, including German doctors, have...

ganz viel Geld verdient und das läuft heute auch noch.

made a lot of money and that is still going on today.

Deutsche Ärzte gehen am Wochenende nach London und behandeln da englische Patienten auf Privatkassen und verdienen da echt viel Geld.

German doctors go to London on weekends and treat English patients on private insurance, earning a lot of money there.

Oder britische Patienten gehen nach Europa und lassen sich da behandeln.

Or British patients go to Europe and have treatment there.

Bezahlen das entweder selber oder sogar der National Health Service bezahlt das, weil die eigene Infrastruktur nicht so ausgebaut ist.
