Wenn das so schleichend beginnt, heißt das auch, dass viele damit gar nicht erst mal zum Arzt gehen, also nicht so schnell?

Also bei uns gilt eine Faustregel, die lautet, dass der Patient frühestens dann zum Arzt geht, auch zum Hausarzt,

Mit welchen Symptomen, ab wann sollte man denn so achtsam sein und sagen, also jetzt sollte ich doch mal drauf schauen lassen?

What symptoms should one be particularly aware of, and when should one say, "I should get this checked out"?

Eigentlich müsste man nur drei Fragen stellen, nämlich erstens, wie alt ist der Patient?

Actually, one should only ask three questions, namely first, how old is the patient?

Sie haben vorhin schon gesagt, über 40.

Nimmt die Häufigkeit dramatisch zu?

Die Erkrankung beginnt häufig schon früher, aber sie macht halt am Anfang keine wirklichen Symptome,

außer vielleicht mal ein bisschen Husten, wenn man einen Infekt hat.

Aber ab 40 steigt das Risiko steil an.

Und wer Rauchen konsequent aus dem Weg geht und Luftschadstoffen konsequent aus dem Weg geht,

hat selbst dann, wenn er andere Risikofaktoren hat, eine gute Chance, dass er nicht wirklich erkranken wird.

Dann hat er halt doch ein relativ hohes Risiko.

Und deswegen können wir nur empfehlen, gerade jungen Menschen, dass man es rauchen lässt.

Dr. Bartschok, was bedeuten diese vier Buchstaben COPD eigentlich ganz genau?

Dr. Bartschok, what do these four letters COPD actually mean exactly?

Ja, das ist mit ein Grund, warum sich der Name einfach nicht so durchsetzt.

Yes, that's one reason why the name just isn't catching on.

Das sind englische Wörter.

These are English words.

COPD steht für Chronical.

COPD stands for Chronic.

Obstruktiv pulmonale Disease.

Obstructive pulmonary disease.

Pulmonale, also Lungen Disease, Krankheit.

Pulmonary, also lung disease, illness.

Und chronisch obstruktiv heißt, dass die Bronchien chronisch, also dauerhaft, verengt sind.

And chronic obstructive means that the bronchi are chronically, or permanently, narrowed.

Das ist übrigens auch der Unterschied zum Asthma.

That is also the difference from asthma, by the way.

Beim Asthma ist es so, dass die sind zumindest am Anfang die Bronchien normal.

In asthma, it is the case that at least at the beginning, the bronchi are normal.

Und dann hat man meinetwegen einen Kontakt mit Allergenen, irgendwelchen Pollen oder Haustabmilben oder was auch immer.

And then you have, for my part, a contact with allergens, some pollen or house dust mites or whatever.

Dann ziehen sie sich zusammen.

Then they come together.

Und man kriegt einen Asthmaanfall.

And you get an asthma attack.

Aber anschließend, wenn das behandelt wird und vielleicht so etwas sensibilisiert wird und man das Allergen meidet,

But afterwards, when that is addressed and perhaps something like that is sensitized and one avoids the allergen,

dann erholen sich die Bronchien wieder und sind wieder normal.

Then the bronchi recover and return to normal.

Da kann man in der Lungenfunktion, sieht man da nichts.

There is nothing to see in the lung function test.

Bei der COPD ist es anders.

With COPD, it's different.

Durch die chronische Vergiftung der Bronchien mit irgendwelchen Luftschadstoffen gehen die schön langsam kaputt,

verengen sich, bleiben auch eng.

Man kann sie schon mit Medikamenten dann ein bisschen erweitern und so.

Und dadurch gehen dann auch die Lungenbläschen kaputt.

Und wenn die Lungenbläschen kaputt gehen, dann geht eben die gesamte Lungenleistung immer mehr nach unten.

Und irgendwann reicht es halt dann nicht mehr.

Und zwar doppelt.

And indeed double.

Zum einen sind die Bronchien verengt, zum anderen fehlen Lungenbläschen.

On one hand, the bronchi are constricted, on the other hand, there are no alveoli.

Und das ist dann natürlich besonders schlimm und besonders schwierig auch zu behandeln.

Die Symptome haben Sie vorhin schon.

Beim Asthma ist es ja so, dass man auch völlig aus der Ruhe heraus plötzlich Atemnot bekommt.

Das ist bei der chronisch obstruktiven Lungenerkrankung ja ein bisschen anders.

That's a bit different with chronic obstructive pulmonary disease.

Genau, das kommt erst ganz am Schluss.

Exactly, that comes at the very end.

Die COPD entwickelt sich langsam über 10, 20, 30 Jahre.

COPD develops slowly over 10, 20, 30 years.

Am Anfang nichts.

In the beginning, nothing.

Am Anfang merkt man nichts, hat man vielleicht ganz minimale Einbußen in der Lungenfunktion, die man aber im Alter gar nicht merkt.

At the beginning, you notice nothing; you may have very minimal losses in lung function, but you don't notice them at all as you age.

Und dann kommt typischerweise Belastungsatemnot, wenn man sich stark belastet, dass man dann eben Beschwerden hat.

And then typical exertional shortness of breath occurs, especially when one exerts oneself heavily, resulting in discomfort.

Und dann sagen viele, ah ja gut, ich bin halt älter oder ich habe einen Bauch und deswegen schlafe ich nicht so gut.

And then many say, oh yeah, well, I'm just older or I have a belly and that's why I don't sleep so well.

Aber in Wirklichkeit sind es halt die Bronchien.

But in reality, it's just the bronchi.

Und was auch früh dazu kommt, ist Husten.

And what also comes early is a cough.

Gerade wenn es um Luftschadstoff geht, um Rauchen, da ist es so, wir haben eine Müllabfuhr in unseren Bronchien.

Especially when it comes to air pollutants and smoking, we have a garbage collection service in our bronchi.

Das sind so Flimmerhärchen in den Bronchien überall, die schlagen von unten nach oben und bringen Schleim und Dreck und alles, was wir so einatmen, dauernd nach oben, wie halt so, ja, wie so ein Förderband, könnte man sagen.

These are tiny cilia in the bronchi everywhere, they beat from the bottom to the top and constantly bring mucus and dirt and everything we inhale upwards, like, well, like a conveyor belt, one could say.

So, wenn man jetzt zum Beispiel raucht, ist es so, dass dieses Förderband stehen bleibt.

So, if you smoke, for example, this conveyor belt comes to a standstill.

Eine Zigarette, wenn man eine Zigarette raucht, hat man acht Stunden keine Müllabfuhr.

A cigarette, when you smoke a cigarette, you have no garbage collection for eight hours.

So, und in der Zeit sammelt sich dann Schleim und Dreck.

Und wenn die Bronchien merken, dass irgendwo da was hängt.

Die Lunge muss ja den Schleim zum Beispiel von ganz unten, vom Zwerchfell, ja, zum Mund hin, sind es 50 Zentimeter Höhenunterschied, wenn sie sitzen oder stehen.

The lungs have to move the mucus, for example, all the way from the bottom, from the diaphragm, to the mouth, which is a height difference of 50 centimeters when you are sitting or standing.

Und das muss ja irgendwie dann hochkommen.

And that has to come up somehow.

Und dann husten wir halt.

And then we just cough.

Also Husten und Belastungsatemnot, das sind die ersten Symptome, die man spüren kann.

So coughing and exertional shortness of breath are the first symptoms that one can feel.

Die aber dann von vielen Patienten einfach nicht ernst genommen werden.

But then many patients simply do not take them seriously.

Ja, da sagt.

Yes, that's what I'm saying.

Da sagt man halt, ja gut, wie gesagt, die Belastung, ja, ich bin halt alt oder ich bin dick oder wie auch immer.

You just say, well, as I said, the strain, yes, I'm old or I'm fat or whatever.

Und beim Husten sagt man, ja, ist halt der Raucher husten.

And when it comes to coughing, people say, well, that's just smoker's cough.

Alle Raucher haben Husten, also habe ich auch Husten und nehme mich auch nicht ernst.

All smokers cough, so I cough too and don't take myself seriously either.

Und dann kommt man immer tiefer hinein, bevor dann wirklich was passiert und man endlich mal irgendwie auf einen Arzt trifft.

And then you go deeper and deeper in before something really happens, and you finally somehow run into a doctor.

Wie wird denn, wenn man dann beim Arzt ist, COPD diagnostiziert?

How is COPD diagnosed when you are at the doctor's?

Ganz einfach.

Quite simple.

Man könnte.

One could.

Bei einer schon fortgeschrittenen COPD könnte man es schon hören.

In an advanced stage of COPD, one could already hear it.

Leider ist die Auskultation, also das Abhören mit so einem Rohr, mit so einem elastischen Röhrchen, ist aus der Mode gekommen.

Unfortunately, auscultation, that is, listening with a tube, with a flexible tube, has fallen out of fashion.

Die jungen Ärzte lernen das gar nicht mehr richtig.

The young doctors are not learning that properly anymore.

Dann würde man nämlich hören, dass es da so rasselt und schleimt und dass da irgendwie Quietschen dabei ist und so.

Then you would hear that it rattles and slimes and that there is somehow squeaking and so on.
