And today we are going to talk here about lymphoma and whether surgery is indicated in cases of lymphoma.

Que o linfoma é um câncer do sistema linfático nosso, né?

That lymphoma is a cancer of our lymphatic system, right?

Mais especificamente dos linfócitos, que é um tipo de glóbulo branco

More specifically of lymphocytes, which are a type of white blood cell.

responsável pela defesa do nosso corpo.

responsible for the defense of our body.

E esse sistema linfático, ele está presente em todo o corpo.

And this lymphatic system is present throughout the body.

Quando a gente tem alguma infecção, alguma agressão,

When we have some infection, some aggression,

ao organismo, esses linfócitos se multiplicam para combater essa infecção.

In the body, these lymphocytes multiply to combat this infection.

O gânglio pode crescer, porque eles estão aumentando de número ali dentro.

The ganglion may grow because they are increasing in number inside.

Eles combatem a infecção, mas logo eles param de crescer e o gânglio diminui.

They fight the infection, but soon they stop growing and the lymph node decreases.

Mas quando esses linfócitos, por algum motivo, alguma mutação,

But when these lymphocytes, for some reason, some mutation,

eles não param de crescer nunca mais, eles viram um câncer

they never stop growing anymore, they become a cancer

e o câncer dos linfócitos é chamado linfoma.

Lymphocyte cancer is called lymphoma.

Doutor, e a cirurgia, principalmente a retirada dos gânglios,

pode ser indicada para esse caso?

No linfoma, a cirurgia tem muito pouco papel.

In lymphoma, surgery plays a very small role.

Esses linfócitos do câncer, do linfoma, saem de uma casinha, vão para outra,

These cancer lymphocytes, from the lymphoma, leave one little house and go to another.

de um gânglio migrando para outra.

from one ganglion migrating to another.

Mesmo, às vezes, a gente tendo só um gânglio que a gente vê

Even sometimes, we only have one lymph node that we can see.

ou que aparece nos exames do PET ou da tomografia,

or that appears in the PET scans or the CT scan,

a gente tirar esse gânglio dá a falsa impressão de que a gente tratou,

removing this node gives the false impression that we treated it,

porque esses filhos, esses linfócitos,

because these children, these lymphocytes,

esses linfócitos do câncer podem já estar em outros lugares,

these cancer lymphocytes may already be in other places,

ou próximo, ou do outro lado.

either nearby or on the other side.

E aí, a gente tem a recidiva do linfoma.

And then we have the recurrence of the lymphoma.

Então, tratar com cirurgia, na maioria das vezes,

So, treating with surgery, most of the time,

exceto em situações muito específicas,

except in very specific situations,

não é um bom tratamento porque o linfoma volta depois.

it is not a good treatment because the lymphoma comes back afterwards.

No início, eu tinha essa dúvida e eu também já escutei muito durante o tratamento.

At first, I had this doubt and I also heard it a lot during the treatment.

E a cirurgia? Você não vai fazer cirurgia? Não pode?

And the surgery? Aren't you going to have surgery? Can't you?

Não pode.

Cannot.

As pessoas tendem até a achar que é um câncer,

People even tend to think it's cancer,

que já está com a metástase,

that already has metastasis,

ou é incurável por conta disso,

or it is incurable because of this,

porque elas estão acostumadas sempre com a cirurgia também,

because they are always used to the surgery too,

em alguns casos, na maioria das vezes.

In some cases, most of the time.

É outra diferença que a gente não tem nos linfomas.

It's another difference that we don't have in lymphomas.

A gente não tem essa palavra metástase com linfoma, né?

We don't have the word metastasis with lymphoma, right?

Metástase é quando o câncer saiu do órgão dele e se alojou em outro lugar.

Metastasis is when cancer has spread from its original organ and settled in another location.

No linfoma, como o sistema linfático está em todo o corpo,

In lymphoma, since the lymphatic system is throughout the body,

não é metástase quando ele muda os lados,

it is not metastasis when it changes sides,

entendeu?

Did you understand?

E no caso da biópsia do linfoma, pode ser considerado uma cirurgia?

And in the case of lymphoma biopsy, can it be considered surgery?

Para que a cirurgia é bom no linfoma? Para dar o diagnóstico.

What is surgery good for in lymphoma? To provide the diagnosis.

Nós temos mais de 40 tipos diferentes de linfoma,

We have more than 40 different types of lymphoma,

que a gente trata às vezes de jeito diferente,

that we sometimes handle in a different way,

o comportamento também é diferente de um para o outro.

the behavior is also different from one to another.

Só o médico patologista olhando dentro desses gânglios dos linfonodos

Only the pathologist can look inside these lymph node glands.

para saber qual é o tipo de linfoma dele.

to find out what type of lymphoma he has.

Isso muda muito. Isso é muito importante para a gente.

This changes a lot. This is very important to us.

Muda o tratamento?

Does the treatment change?

Muda para o diagnóstico.

Switch to the diagnosis.

Sem biópsia, não se faz diagnóstico de linfoma.

Without a biopsy, lymphoma diagnosis cannot be made.

E muitas vezes ele é um procedimento cirúrgico.

And often it is a surgical procedure.

A gente pode fazer uma pequena cirurgia no local onde está o linfonodo,

We can do a small surgery on the spot where the lymph node is.

retirar ele inteiro, é melhor no pulmão, no fígado,

removing it whole is better in the lung, in the liver,

fazer um pequeno procedimento cirúrgico para tirar um pedacinho desse tecido

perform a small surgical procedure to remove a small piece of this tissue

e ver se ele é um linfoma.

and see if it is lymphoma.

Mas muitas vezes a biópsia hoje em dia não é feita por cirurgia.

But many times the biopsy nowadays is not done through surgery.

Ela é feita por agulha guiada por tomografia, na maioria das vezes.

It is done by a needle guided by tomography, most of the time.

Então, a gente vai

So, we are going.

com uma agulha mais grossa um pouquinho e tira um pedacinho pela agulha

with a slightly thicker needle and take a small piece through the needle

para fazer o diagnóstico.

to make the diagnosis.

E isso não dá diagnóstico de linfoma, a gente não consegue dizer.

And this does not provide a diagnosis of lymphoma, we cannot say.

Então, realmente, é uma agulha um pouquinho mais grossa que o médico usa.

So, really, it is a needle that is a little thicker than the one the doctor uses.

E agora esse risco de espalhar para o linfoma não existe, de contraindicar.

And now this risk of spreading to lymphoma does not exist, to contraindicate.

Mas pode existir sim para outros tipos de câncer.

But it can exist for other types of cancer.

Então, o que acontece é que às vezes a pessoa está com um gânglio

So, what happens is that sometimes a person has a lymph node.

e o médico diz,

and the doctor says,

o médico não sabe se é realmente um linfoma

the doctor does not know if it is really a lymphoma

ou se não pode ser a metástase de um outro câncer no gânglio.

or it could be the metastasis of another cancer in the lymph node.

Por isso que, às vezes, ele tem medo de fazer,

ou algumas pessoas têm medo de fazer essa aspiração.

Or some people are afraid to make this aspiration.

Se for a suspeita de linfoma, a gente não faz.

If it's a suspicion of lymphoma, we don't do it.

É melhor tirar mesmo o gânglio e, na impossibilidade de tirar,

It's better to remove the gland, and if it's not possible to remove it,

usar a agulha mais grossa, biópsia por agulha grossa.

use a thicker needle, core needle biopsy.

E quais são os tratamentos indicados para o linfoma, já que a cirurgia não é indicada?

And what are the recommended treatments for lymphoma, since surgery is not indicated?

Como é uma doença, como eu disse para você,

As it is a disease, as I told you,

a gente fala sistêmica, que está em todo o nosso organismo,

we speak systemic, which is present throughout our organism,

em todo o sistema linfático,

throughout the lymphatic system,

a gente usa, portanto, normalmente, tratamentos sistêmicos,

we usually use systemic treatments,

que pegam todo o corpo.

that take the whole body.

Ou quimioterapia, que é o mais comum deles,

Or chemotherapy, which is the most common of them,

ou radioterapia em algumas situações.

or radiotherapy in some situations.

E hoje, atualmente, até os tratamentos mais novos,

And today, currently, even the newest treatments,

que a gente chama de terapia alvo,

what we call targeted therapy,

são inibidores só daquele tipo de célula, daquele linfoma.

They are inhibitors specific to that type of cell, that lymphoma.

Ou a imunoterapia,

Or immunotherapy,

que é usar anticorpos,

what it is to use antibodies,

usar as células do nosso corpo e treinar elas para tratar o câncer.

use the cells of our body and train them to treat cancer.

E são bons tratamentos, têm ótimos resultados.

And they are good treatments, they have excellent results.

E, na maioria dos linfomas, a gente tem taxas de curas muito boas.

And, in the majority of lymphomas, we have very good cure rates.

Tratamentos muito diversos também e distintos.

Very diverse and distinct treatments as well.

Por isso que é importante fazer o diagnóstico certo.

That's why it's important to make the correct diagnosis.

Achar qual é o tipo de linfoma,

Find out what type of lymphoma it is,

que aí sim a gente pode fazer o melhor tratamento para ele.

that then we can provide the best treatment for him.
